The Swindon Serious Case Review Process
From January 2008, Swindon LSCB is making a seventy seven page guide documenting all aspects of the Swindon Serious Case Review Process available to other LSCBs.
The guide draws together all the information developed by Swindon over the last three years to support those involved in carrying out serious case reviews. Section one provides an updated version of practice guidance originally developed by Swindon ACPC; section two lists actions by role, from the LSCB itself, to the Investigating Manager for an agency, the LSCB Business Manager and more; section three gives detailed help on the use of serious case review documents; and section four offers practical administrative tips, for example on merging chronologies and ensuring that forms are correctly completed.
In addition, the appendices include all document pro-formas and standard letters, as well as a slide presentation used to brief investigating managers and a leaflet for families involved in the process.
Although these documents have been developed primarily to support our own process, there was such interest in them at the National LSCB event last March that we have been encouraged to make them available to other LSCBs. They were also used as the basis for the first Swindon and Wiltshire Domestic Homicide Review.
No process is perfect, so we expect our guide to reflect this by continuing to evolve. If other LSCBs use all or part of our process, we hope very much that they will contribute to this evolution by sending us their feedback and ideas for improvement.
